Legal Forms Clinic to assist with filling out forms, and e-filing or making appointments as needed. All forms will be provided. There will be an attorney and notary on hand.
Assistance will be available to individuals who are 18+ and accompanied minors.

Assistance will be for:
Legal name changes ***Notary required to submit application***
Petition to waive publication of legal name changes
Passport applications
For folx who have already completed their legal name and/or gender marker changes, assistance will be available for updating the following documents:
Social Security Card
State issued ID/license
Existing passport
Birth certificate (as permitted by various states’ laws) ***Notary required to submit application***

Applicants for legal name changes will need to present:
Certified birth certificate*
Legal ID such as current state-issued IDs, drivers licenses, or passports
Any documentation of previous legal name changes if applicable

Applicants for updating legal name and/or gender marker changes on birth certificates will need to present:
Certified birth certificate*
Legal ID such as current state-issued IDs, drivers licenses, or passports
Court order for legal name and/or gender marker changes
The other forms will require ID, birth certificates, proof of residency, and court orders when submitted to the appropriate agency for processing, but folx will not need to bring those documents to our clinic to fill out forms. Different forms and/or additional documentation may be required for filing on behalf of a minor.
If you are missing a birth certificate or other necessary documentation but would like to prepare forms anyway you can still receive assistance with filling out forms.
